And indicate the severity of reaction to each of the allergens.  Use a scale of 1-10 with 1 = very mild, hardly noticeable to others and 10= life threatening if emergency attention is not received immediately.

Are you willing to accept responsibility for any/all reaction(s) you and/or your family members may experience during the allergy trial?  In other words, you will not hold the American Hairless Terrier Association,  referring breeder and/or AHT owner responsible for any illness or injury that may result from your visit and subsequent exposure to the allergens in the environment.
